The stars on the d20s represent the highest and lowest rolls in case the GM wants to restrict crticial successes and failures to a 1 in 20 chance vs the larger range these dice will offer.

The dice can be used as a real in game magic item that a player can find. Instead of rolling a D20, replace the roll with a die you blindly pull from the pouch. You must use the new D20 and it is not returned to the pouch. When all 8 dice are used, the magic item vanished to somewhere else in the world.

OR ... a DM can use the dice by themselves to give to players to represent blessing and curses that might have happened during the game. Since the modifed dice range from -5 to +5 ... there is a die fitting many a blessing and curse that might befall a player.

https://www.thediceshoponline.com/products/impact-tyches-pouch-of-mixed-fortune-8-pre-modified-d20-dice
